How To Become a Provider Agency (including a Program Approved Service Agency (PASA)
Step 1 – Letter of Intent (LOI)
Submit a Letter of Intent (LOI) to CDPHE. CDPHE will send the formal application packet. Once deemed complete, an on-site survey will be scheduled. Timing varies – always check CDPHE’s PASA page for current scheduling estimates.
Step 2 – CDPHE Application & On-Site Survey
- CDPHE will review your application and policies/procedures (incident reporting, staff training, background checks, emergency preparedness, monitoring schedules).
- A CDPHE on-site survey is required for compliance verification.
Step 3 – HCPF Medicaid Provider Enrollment
You may begin HCPF enrollment during the CDPHE process. However, HCPF Operational Memo OM 24-001 (effective Jan 5, 2024) requires that any specialties needing CDPHE certification must have certification attached to the HCPF application. *Applications submitted without certifications will be returned or denied.
Step 4 – National Provider Identifier (NPI)
- NPI strongly recommended.
- Confirm if required for your service specialties via the HCPF HCBS Provider Specialty Code List.
Step 5 – Specialty Codes & Licensing/Certification
Table of specialties/licensure included in the download document.
Always confirm the most current requirements on the HCPF HCBS Specialty Code List.
Step 6 – COHFI Portal & Attachments
Download certifications from COHFI portal, attach to HCPF applications.
*Mandatory per OM 24-001 – HCPF applications without COHFI certifications (or address-change screenshots for certification-only providers) will be returned or denied.
Step 7 – Final Approval
- After CDPHE approval and HCPF enrollment (with certification attached), HCPF issues final approval.
- Only then can referrals be made by Case Management Agencies (CMAs).

- Decide specialties/services → Use HCPF HCBS Specialty Code List.
- Submit Letter of Intent (LOI) to CDPHE → CDPHE PASA Page.
- Prepare policies/procedures → incident reporting, monitoring, staff qualifications, emergency/disaster plan, background checks.
- Complete CDPHE application → await review and on-site survey (timing varies).
- Register in HCPF Provider Web Portal → HCPF Provider Enrollment.
- Download certifications from COHFI portal once CDPHE approves → attach to HCPF application.
- Submit HCPF enrollment application → one application covering all specialties; attach certifications (required by OM 24-001).
- Obtain NPI if required by your specialty.
- Complete fiscal agent (DXC) site review if applicable.
- Receive HCPF final approval → Only then can CMAs begin referrals.
