Case Management Redesign in Colorado includes many changes, one of them being the Case Management Agency (CMA) transition. All home and community-based case management services are being combined under one contract in each defined service area to better support the people we serve. The following information outlines ongoing phases as part of this transition that may be impacting the people we serve in our service area as well as across the state. Helpful information is also included at the bottom to keep you informed with resources.
CHCBS Transition Phase
beginning May 2024
Families will gradually transition to TRE using a phased approach and all CHCBS families should anticipate being fully transitioned to TRE by July 1, 2024. During this transition, a new service coordinator will be assigned to your family.

What This Means
Q: Will my child’s current services be affected?
A: No. Any needed changes will continue to be made through the traditional review process.
Q: Who will provide Case Management for children enrolled in the CHCBS Program?
A: If your child is enrolled in CHCBS, your child will receive a new Service Coordinator. TRE has employed and contracted Service Coordinators who are familiar with the CHCBS program.
Q: My child’s current Private Case Management Agency will no longer be providing Case Management Services. How will my child be impacted?
A: If your child’s current agency is no longer providing Case Management Services/Service Coordination, your child will be assigned a new Service Coordinator TRE.
Q: How can I find out who my CHCBS Service Coordinator is?
Q: If our child is currently receiving IHSS, will that service be impacted?
A: No, the service will remain the same, unless a revision is warranted.
Q: How can I refer my child for the CHCBS Waiver?
A: Beginning July 1, 2024, TRE will process all referrals and intake for the CHCBS waiver, along with all other waivers within El Paso, Park, Teller, and Pueblo Counties.Q: Who determines if my child is eligible for CHCBS?
A: Although TRE is the central referral and intake entity for all waivers, other entities are involved in determining eligibility and it is a multi-step process. After the assessment is completed, information is evaluated by the third-party reviewer. Additionally, financial eligibility is reviewed by outside entities. This information is sent back to TRE and your Case Manager schedules an enrollment meeting. Depending on your child’s needs, you may also enroll into IHSS services. This initial process can take several months due to the various entities involved.
Pueblo County Transition Phase
beginning March 2024
TRE was awarded the CMA contract with the Colorado Department of Health Care Policy and Financing (HCPF) to provide case management services to people enrolled or enrolling into all Long-Term Services and Supports (LTSS) in Pueblo County. TRE assumed these case management responsibilities March 1, 2024.
A coordinated process began to transition current members and their case management services for Community Centered Board (CCB) and Single-Entry Point (SEP) waivers to TRE.
What This Means
Q: Do members need to take action because of this change?
A: No. There is no action members need to take. TRE and our partners are working together to transition case management services.
Q: Are services received as outlined in service plans going to change?
A: No. Services will be the same.
Q: Are any services remaining with Pueblo County DHS?
A: Yes, Pueblo County DHS is subcontracting with TRE and will continue to provide:
-Intake, to include Nursing Facilities, PACE, and Hospital back-up for all non-IDD waivers (Persons with Brain Injury, Community Mental Health Supports, Elderly Blind and Disabled, Complementary and Integrative Health, and Children with Life Limiting Illnesses waivers).
-Case Management Services to:
All members residing in an Assisted Care Facility
All members receiving In-Home Services and Supports (IHSS)
All members receiving Consumer-Directed Attendant Support Services (CDASS)

For those members who do not fall into one of the categories above, The Resource Exchange will send communication as to who your new Case Manager (Service Coordinator) may be.
Q: Will all members get a new case manager?
A: Because a case management agency is changing, members may have a new case manager. Members will receive information from TRE including case manager’s contact information. However, TRE is working collaboratively with our partners to ensure the least amount of disruption to the member. Should a new case manager be assigned to you, The Resource Exchange will send communication (letter or email) as to who your new Case Manager (Service Coordinator) will be.
Q: I haven’t received a letter yet. Should I expect one soon?
A: Letters are being distributed, either electronically or by regular mail. If you have not received a letter, please contact our Pueblo office at 719-601-9991 or toll free at 877-423-0323 and we will follow up with you.
- If you haven’t already picked up your bus pass in person at the Colorado Bluesky offices, they are available for pick up at our new Pueblo office located at 201 W. 8th Street Suite 600.
- If you receive your bus pass by mail, they have already been mailed out.
- If you pick up your bus pass at a provider agency, please continue to visit your provider agency.

Colorado’s Long-Term Services and Supports (LTSS) system is being impacted by several challenges at the same time, including:
- Increased county workload due to the end of the Public Health Emergency (PHE);
- The implementation of a new Care and Case Management IT system with issues that have required additional work for case managers; and
- The transition of members to new case management agencies (CMAs) to achieve conflict-free case management.
The impact of all three occurring at once – which was not intended – is causing short-term challenges to member program eligibility, provider reimbursement, CMA processing and member service response time.
