Children's Extensive Support (CES)
Medicaid Waiver Program

PURPOSE:
The Children's Extensive Support (CES) Medicaid Waiver Program provides services to children who have a developmental disability (or, for under age five, have a developmental delay) who are at risk of institutionalization. The services provided under this program serve as an alternative to institutional services for children from birth through seventeen years of age who meet the targeting criteria and the Level of Care Screening Guidelines. This program is designed for only a small number of eligible children. It is administered by The Colorado Department of Human Services, Division for Developmental Disabilities, with oversight by Health Care Policy and Finance.

ELIGIBILITY:

The child has not reached his/her 18 th birthday; and
The child is living at home with his/her biological, adoptive parent(s) or guardian, or is in an out-of-home placement including an Intermediate Care Facility for the Mentally Retarded (ICF/MR), hospital or nursing facility and can be returned home with the provision of CES services; and
The child, if age five or older, has a developmental disability; or if less than five years of age, has a developmental delay, as determined by a community centered board (CCB – for El Paso, Park and Teller Counties, the CCB is The Resource Exchange); and
Children enrolled in this 1915(c) waiver shall be eligible for Supplemental Security Income (SSI); and
The quality and quantity of medical services and supports identified in the Individualized Plan (IP) are provided pursuant to a physician’s order to meet the needs of the child in the home setting; and
The income of the child shall not exceed 300% of the current maximum SSI standard maintenance allowance; and
The resources of the child shall not exceed the maximum SSI allowance; and
Enrollment of a child under this rule shall result in an overall savings when compared to the ICF/MR cost as determined by the State; and
The State’s Utilization Review Contractor (URC) certified that the child meets the Level of Care for ICF/MR placement; and
The child demonstrates a behavior or has a medical condition that require direct human intervention, more intense than a verbal reminder, redirection or brief observation of medical status, at least once every two hours during the day and on a weekly average of once every three hours during the night. The behavior or medical condition must be considered beyond what is typically age appropriate.

SERVICES AND SUPPORTS:
In addition to providing Medicaid services to the enrolled child, the following services are available:

Personal Assistant Services – include personal care, household services, childcare, respite care, sibling care.

Community Connection Services – recreational and leisure services, connections to the community.

Professional Services – services provided above the Medicaid State Plan benefits. Professional Services are those services, including evaluation and assessment, which require the service providers to be licensed or certified in a particular occupational skill area.

Behavioral Services – provided when challenging behaviors interfere with activities of daily living, social interactions, or other similar situations. Services include diagnostic evaluations, consultations with family members and professionals providing services, setting up behavior plans to be implemented by others, counseling services, behavioral interventions related to the child’s disability.

Home Modifications Services – services include accessibility modifications (ramps, railings, etc.) and safety (alarms, intercoms, etc.).

Assistive Technology Services – such as communications devices, safety devices, skill acquisition, mobility devices, adaptive recreational equipment. Also includes repairs or maintenance of such devices, as well as AT assessment and training.

Specialized Medical Equipment and Supplies – including durable medical equipment as well as non-medical, general care items (ointments, lotions, etc.), wipes, dental swabs, also specialized clothing. Repairs or maintenance of medical equipment, and assessment and training.
